| Once you come through our entryway you will find our office suite straight back on the right. New patients are asked to complete a registration form at their first appointment. This form can be downloaded and completed for you to bring to your first visit. Please come a few minutes early. It is helpful to bring all of your medications and any old records (especially immunizations) to your first visit. The fee for your initial visit is slightly higher than our usual fee because of the extra time involved. | |
